How do i know if i seriously injured my hand/wrist?

well this afternoon around 1pm i was closing the sliding door of my parents van door really hard and my right wrist/hand got slammed in there i had instant bruising and swelling and excruciating pain that gets worse with movement. i can move my hand ok but its EXTREMELY painful when i do move it. it is very very sensitive to the touch and hurts really really bad to touch it there are some parts that feel "lumpy". i dont know if i seriously injured my hand/wrist or not. it is now 10:20 pm the same day and the pain has moved into my forearm. not really bad but noticible in my forearm. ice doesnt make it any better.
